This directory has been compiled to assist health care professionals in locating cardiovascular and pulmonary rehabilitation programs available within geographical areas of the state. We encourage patients to contact programs available in their areas and choose the one that meets their particular needs. However, because of variations among insurers, patients should call their health insurance providers to discuss the programs and locations available to them.

Cardiac Rehabilitation

Cardiac Rehabilitation is intended to assist persons with heart disease. If individuals have any of the following conditions, they may be candidates for cardiac rehabilitation:

  • Stable Angina
  • Heart Attack (Myocardial Infarction)
  • Cardiac Surgery  (Coronary Artery Bypass Graft (CABG), Valve Replacement and/or repair)
  • Percutaneous Transluminal Coronary Angioplasty (PTCA)
  • Stent Placement
  • Heart Failure patients who meet criteria
  • Peripheral Artery Disease

Pulmonary Rehabilitation

Pulmonary Rehabilitation is intended to assist persons with lung disease. Anyone with the following conditions, or other chronic lung conditions, may be candidates for pulmonary rehabilitation:

  • Cystic Fibrosis
  • Lung Cancer/surgery
  • Restrictive Lung Disease/Morbid Obesity
  • Lung Surgery
  • Pre/Post Lung Transplant
